2023-2027 Community Health Improvement Plan

The Community Health Improvement Plan (CHIP) is a roadmap and action plan for how the Health Department will work alongside community members and partners to address some of the most important health issues in the West Allis, Greenfield, and West Milwaukee communities.

In April of 2024, the West Allis Health Department and the Greenfield Health Department merged, forming the Southwest Suburban Health Department (SWSHD). Prior to the merge, each department completed a community health assessment and a CHIP for 2023-2027. The SWSHD CHIP for 2025-2027 has integrated these plans and will continue efforts in the following areas of focus: Behavioral/Mental Health, Substance Use, and Violence Prevention & Community Connections. Across all focus areas will be the overarching themes of improving equity and access to resources. Collaborative action teams were established for each area of focus in 2023 by West Allis, and have been expanded to involve community partners from all municipalities served. We invite you to follow along with us as we work to accomplish the below goals and achieve a healthier community.

Behavioral/MentaI Health

Goal 1: Enhance collaboration, understanding, and access to behavioral health resources in the community.

Goal 2: Promote the inclusion and voices of youth in our efforts to address emerging health needs.

Goal 3: Increase awareness and acceptance of behavioral health issues in the 55+ adult population.

Substance Use

Goal 1: Increase education on alcohol misuse and vaping.

Goal 2: Increase understanding of Substance Use Disorder and access to harm reduction services in the community.

Violence Prevention & Community Connections

Goal 1: Encourage and collaborate with innovative approaches that foster an environment of inclusion and diversity.

Goal 2: Connect populations experiencing domestic violence to supportive services and safety planning.

Goal 3: Increase awareness and understanding of healthy relationships and family supports.
