The Community Health Improvement Plan (CHIP) is a roadmap and action plan for how the Health Department will work alongside community members and partners to address some of the most important health issues in the West Allis and West Milwaukee community. Based on findings from the 2022 Community Health Assessment, the three areas of focus for the 2023-2027 CHIP are Behavioral/Mental Health, Substance Use, and Community Safety & Violence Prevention. Action teams were established for each area of focus, made up of community partners that helped create the goals, objectives, and strategies. The below goals along with the CHIP will evolve over time, just as the needs of the West Allis and West Milwaukee community do.
