how to request a birth or death certificate

1. Make an in person appointment at the West Allis Health Department by calling 414-302-8600.

  • Appointments are available Monday- Thursday from 8:30 a.m. - 3:30 p.m. 
  • Typical appointments take under half an hour and you will leave with your certificate in hand. 
  • Cash and checks only accepted as payment in person 

2. Complete an application online here.  

  • Orders will  be processed once payment is received. All identification information must be received in order to process the application. 
  • Credit card payments are accepted for online orders (with additional fee). Cash and checks can be dropped of at the Health Department during normal business hours 8:00am-5:00pm Monday-Friday. 
  • Checks can be mailed to: 7120 W. National Ave. West Allis, WI 53214.
  • Please allow 2-3 days to process online applications. 
  • You will be able to choose pick up or mail (no extra charge) for delivery of your certificate.  


The first copy of all vital records is $20.00.  Additional copies ordered at the same time as the original request will be $3.00 each.  

Statewide Issuance

We are able to issue birth certificates for any births in the State of Wisconsin taking place after October 1, 1907 and death certificates for any deaths in the State of Wisconsin taking place after September 1, 2013.

Important Application Information

Please read the application carefully to see who is authorized to obtain a birth or death certificate. 

If you are NOT a member of the IMMEDIATE family as listed on the application, you may only obtain the certificate under the following circumstances:

  • You have a written and notarized authorization clearly stating the relationship of the authorizing party to the person named on the record. 
  • You provide documentation that there is a legal purpose for obtaining the record.
