WAWM Community Health Improvement Plan 2016-2020

This community driven process was conducted to determine and rank health priorities and develop a blueprint for community action over the next 5 years for the City of West Allis and Village of West Milwaukee. The CHIP process began by convening a diverse group of 24 community leaders and stakeholders to review the most pertinent data trends from the WAWM Community Health Assessment (CHA) 2015 and choose the priority issues to be addressed in the CHIP. An action team of community partners was then convened for each of these priority areas to develop goals, objectives and strategies related to the issue.

The priority issues that were selected are: 

  • Healthy Lifestyles
  • Mental Health  
  • Substance Abuse and Addiction
